Applicant Name : Please enter your name as the applicant.
MOBILE NUMBER : Provide your valid mobile number without the country code.
EMAIL ID : Enter your email address correctly. This will be used for sending the certificate and acknowledgment.
OFFICE ADDRESS : Ensure to include your complete office address, including the state and postal code.
SOCIAL CATEGORY : Select your social category from the available choices.
AADHAAR NUMBER : Applicants must provide their 12-digit Aadhaar number issued by UIDAI
PAN CARD NUMBER : Applicants must provide their PAN card number. (For proprietorships, provide the owner's PAN number).
BANK ACCOUNT NUMBER : Enter the bank account number as listed on your passbook.
IFSC CODE : Provide the IFSC code from your passbook.
DATE OF COMMENCEMENT OF BUSINESS : Enter the date your company was incorporated or registered, as this will appear on your Udyam certificate.
TYPE OF ORGANIZATION : Choose your organization type from the available options, which will be reflected on your certificate.
MAIN BUSINESS ACTIVITY OF ENTERPRISE : Choose the primary activity of your business from the options provided.
ADDITIONAL DETAILS ABOUT BUSINESS : You may provide further information about your business here.
NUMBER OF EMPLOYEES : Indicate the total number of active employees in your organization.
INVESTMENT IN PLANT & MACHINERY / EQUIPMENT : State the total investment in plant, machinery, and equipment made for starting your business.
Upload Pan Card : Applicants can upload a scanned copy of the front side of their Pan card (jpg, png file < 12MB).
SUBMIT APPLICATION : Click the submit button after providing all required details and documents.
